Job Type: Full-Time
Schedule: Monday-Friday, with occasional evenings or weekends as needed
 
We are seeking a compassionate, organized, and motivated Case Management Worker to assist individuals in achieving personal stability and success. The Case Management Worker will provide support, advocacy, resource coordination, and case documentation while helping clients navigate housing, employment, education, healthcare, and community services.
 

Qualifications

  • High school diploma or GED.
  • Human services experience preferred.
  • Valid driver's license preferred.

Responsibilities

  • Assess client needs.
  • Connect clients to resources.
  • Maintain case files.
  • Monitor client progress.
