Responsibilities:

• Coordinate health and wellness initiatives, screenings, referrals, and follow-up care
• Develop and maintain Individual Health Care Plans and health documentation
• Provide training and support to staff, families, and providers
• Track health requirements, referrals, and program compliance
• Collaborate with healthcare providers, community organizations, and internal teams
• Maintain program records, reports, and databases
• Support community outreach, advocacy efforts, and professional development activities

Qualifications:

• Current NYS licensure as an RN, NP, PA, or MD
• 3-5 years of experience in healthcare, public health, case management, or a related field
• Bachelor's degree or equivalent healthcare education required
• Strong communication, organizational, and customer service skills
• Experience working with children, families, or community-based programs preferred
• Valid driver's license and ability to travel locally as needed
• Ability to pass a criminal background check
