Join our client's dedicated team as a Case Manager, where you will play a vital role in welcoming immigrants with limited English proficiency in the Rockford area. Your work will help them access the comprehensive services they need to build a new life.

 

Responsibilities:

• Welcome and provide support to newly arrived immigrants and refugees
• Assess client needs and create individualized service plans
• Assist with referrals to public benefits, housing, healthcare, and employment services
• Provide language support and resources to help navigate community services
• Collaborate with other organizations and agencies for additional client assistance
• Maintain detailed records and track client progress

Qualifications:

• Experience in social work, case management, or a related field
• Associate’s degree a plus
• Strong understanding of public benefits and community resources
• Compassionate and culturally sensitive approach to client care
• Bilingual in English and Spanish required
• Excellent organizational and communication skills
