We are seeking a compassionate and resourceful Case Manager for a rewarding career opportunity supporting mothers and children in a residential setting. This full-time position plays a critical role in helping residents access community resources, develop stability, and achieve their personal goals through ongoing support and advocacy.

General Responsibilities of a Case Manager

• Provide ongoing case management services and individualized support to residents.
• Connect residents with community resources, benefits, healthcare services, housing assistance, employment resources, and other supportive programs.
• Assist residents in developing and achieving personal goals through advocacy, coaching, and resource coordination.
• Maintain accurate case notes, documentation, and resident records.
• Conduct resident assessments and monitor progress toward established goals.
• Collaborate with internal staff and external service providers to ensure comprehensive support services.
• Occasionally transport residents to appointments, meetings, and community resources as needed.

 

Ideal Background for a Case Manager

• Bilingual Spanish/English required
• Experience providing nonprofit, education/childcare, advocacy, social services support preferred.
• Excellent communication, organization, and interpersonal skills.
• Ability to work independently while collaborating effectively with a team.
• Flexibility and adaptability in a fast-paced environment.
• Proficiency with Microsoft Outlook, Word, and basic Microsoft Office applications.
• Valid driver's license and ability to transport residents as needed preferred.
