Key Responsibilities:

• Schedule and coordinate healthcare appointments, maintaining accurate records of changes and follow-ups.
• Communicate with healthcare providers, nursing staff, pharmacies, and residential teams.
• Maintain medical records, eMAR documentation, PCP information, and compliance logs.
• Support medication management, reconciliation, and distribution processes.
• Assist with OHCQ/DDA and MBON compliance activities and clerical duties.
• Order medical supplies, manage inventory, and verify pharmacy deliveries.
• Attend healthcare appointments and PCP meetings as needed.
• Provide basic first aid, medication administration, and direct support to individuals served.
• Train staff on assistive technology and advocate for the health, rights, and dignity of individuals supported.
• Perform additional duties as assigned.

Qualifications:

• High School Diploma or GED required.
• Valid Maryland driver's license with a clean driving record.
• Proficiency in Microsoft Office.
• Strong organizational, communication, and problem-solving skills.
• Ability to lift up to 50 lbs. and push/pull up to 100 lbs.
• CPR/First Aid, CMT, and Mandt certifications required within 90 days of hire.
• Must complete all required DDA and company training.
