Field Supervisor – Home Care

Position Summary

The Field Supervisor is responsible for overseeing the daily coordination of home care services to ensure quality care, regulatory compliance, and operational efficiency. This position serves as a primary resource for caregivers, office staff, clients, and referral sources by monitoring service delivery, resolving concerns, supporting staff performance, and maintaining compliance with agency policies and applicable state regulations.

 

Essential Duties and Responsibilities

  • Supervise the daily operations of home care services to ensure timely and effective service delivery.
  • Monitor caregiver schedules, staffing assignments, and service continuity to meet client needs.
  • Provide guidance, coaching, and ongoing support to caregivers and office staff regarding agency policies, procedures, and best practices.
  • Collaborate with the scheduling team to address staffing shortages, call-offs, and emergency coverage.
  • Review client service plans and coordinate with clinical and administrative staff to ensure services are delivered as authorized.
  • Respond promptly to client, family, caregiver, and referral source inquiries, concerns, and service issues.
  • Monitor caregiver performance, attendance, documentation, and compliance with agency standards.
  • Assist with onboarding, orientation, and ongoing training of caregivers and supervisory staff.
  • Conduct performance evaluations and recommend corrective actions, recognition, or additional training as needed.
  • Ensure compliance with state regulations, accreditation standards, and agency policies.
  • Review documentation for accuracy, completeness, and timeliness.
  • Assist with audits, quality assurance activities, and performance improvement initiatives.
  • Maintain accurate records related to staffing, client services, incidents, and employee performance.
  • Coordinate communication between departments to promote efficient operations and high-quality client care.
  • Participate in on-call rotations or operational support as assigned.
  • Perform other duties as assigned by management.

Qualifications

  • High school diploma or equivalent required; Associate's or Bachelor's degree in Healthcare Administration, Business Administration, Nursing, or a related field preferred.
  • Minimum of two (2) years of supervisory or leadership experience in home care, healthcare, or a related service industry.
  • Strong knowledge of home care operations, scheduling processes, and regulatory requirements preferred.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficiency with electronic medical records (EMR), scheduling software, and Microsoft Office applications.
  • Ability to maintain confidentiality and exercise sound judgment.
