Job Duties: 

• Manage client cases
• Assess needs
• Set goals
• Document progress
• Connect clients with resources
• Follow up on action plans 
• Monitor risk and stability 
• Make sure that clients receive respectful and timely support 
• Keep records accurate and up to date 
• Customer service 
• Use computers and computer programs 
• Answer phone

Requirements: 

• High School Diploma or equivalent is required 
• Additional education in human services, social work, family services, behavioral health, education or related field is helpful 
• Experience helping individuals or families through customer service, non profit services, community work, healthcare education, case support or similar setting
• Able to communicate clearly and efficiently 
• Able to actively listen and follow verbal and written instructions 
• Organized 
• Good with critical thinking and problem - solving 
• Comfortable working on and with computers and computer programs 
• Ability to sit, stand, walk, bend, reach
• Able to answer multi-line phone
• Excellent customer service and phone etiquette skills 

Work Schedule: 8am to 4:45pm Monday - Friday
