We are seeking a reliable and compassionate Licensed Practical Nurse (LPN) in Taunton, MA to provide part-time in-home check-in visits for patients in the community. This role involves traveling to multiple residences to assess patient well-being, monitor health conditions, and ensure care plans are being followed. Haitian-Creole language skills are highly preferred.

Travel:

• MUST be able to travel from the Cape Cod to Boston areas.
• Compensated for travel expenses

Schedule:

• Part-time, flexible hours
• Travel required between patient homes

Pay: $35.00 per hour

Responsibilities:

• Conduct routine in-home visits to assess patients’ overall health and safety
• Monitor vital signs and report any changes or concerns to the care team
• Ensure patients are following prescribed care plans and medications
• Provide basic nursing care and support as needed
• Maintain accurate and timely documentation of each visit
• Communicate effectively with patients, families, and healthcare providers
• Identify potential risks and report any safety or health concerns

Qualifications:

• Active LPN license in good standing
• Previous experience in home health or patient care preferred
• Strong assessment and communication skills
• Ability to work independently and manage a flexible schedule
• Reliable transportation and valid driver’s license required
