About the Role
Express Employment Professionals is hiring a Case Manager for a short-term assignment focused on Oregon Project Independence (OPI). This role will primarily involve phone-based case management, supporting consumers through service planning and coordination.

This is a great opportunity for experienced individuals in social services who are comfortable working independently, managing client communication, and completing detailed documentation tied to program funding.

Key Responsibilities

• Conduct phone-based outreach and communication with consumers
• Develop and complete service plans (PLANs) based on individual needs
• Maintain accurate and timely documentation for all client interactions
• Ensure compliance with program guidelines and funding requirements
• Collaborate with internal teams to support client services

Qualifications

• Minimum of 4 years of combined education and/or relevant experience in social services or a related field
• Bachelor’s degree in social sciences or related field preferred (experience may substitute)
• Experience working directly with consumers in a social service setting strongly preferred
• Ability to manage sensitive information with professionalism and confidentiality
• Strong communication and organizational skills
• Must pass a basic criminal history check
• Valid Oregon driver’s license or reliable transportation required
