Job Overview

We are seeking a licensed Physical Therapist (PT) for a per diem home health position in Port Richey, FL. This role is ideal for a flexible, patient-focused PT looking to supplement their schedule while providing high-quality care in a home health or clinical setting.

Key Responsibilities

• Evaluate patients’ physical conditions, including mobility, strength, balance, and pain levels, to develop individualized care plans that promote independence at home
• Create and implement personalized physical therapy treatment plans focused on restoring movement, reducing pain, and preventing further injury
• Provide hands-on therapy, therapeutic exercises, gait training, balance activities, and training with assistive devices
• Educate patients and family members on home exercise programs, proper body mechanics, and adaptive equipment use
• Maintain accurate and compliant clinical documentation and communicate effectively with physicians and interdisciplinary healthcare teams
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes
• Ensure compliance with home health regulations, safety standards, and patient confidentiality (HIPAA)

Requirements

• Active Florida Physical Therapist (PT) license
• Minimum 1 year of recent Physical Therapy experience (home health, rehab, acute care, or outpatient preferred)
• Current BLS/CPR certification
• Current TB test
