The Housing Stabilization Case Manager, Housing Retention works as part of a multi-disciplinary team of community partners and staff working together to support low-income participants and families who are experiencing housing instability to gain or maintain housing stability. This includes, but is not limited to, populations such as seniors, individuals with disabilities, and vulnerable neighbors who are unhoused or at risk of becoming unhoused.

The ideal candidate has experience working with issues related to houselessness, substance abuse, mental health, and a history of trauma. This position locates and secures permanent housing opportunities for participants while providing a high level of trauma-informed, coordinated care.

Position Duties & Responsibilities

• Return phone calls, respond to website inquiries, and process and review program applications accurately and in a timely manner
• Interview and screen participants and collaborate with leadership to determine eligibility for federal and state assistance programs (including security deposits, rental assistance, energy assistance, and emergency assistance)
• Maintain a minimum caseload of households enrolled in a housing stabilization program
• Assist participant households in setting short- and long-term goals to achieve greater stability
• Provide support through positive, one-on-one interactions to build trust and strong relationships
• Monitor household progress and track status to help prevent recurrence of houselessness
• Maintain accurate participant files and complete data entry in a timely manner using internal and grant databases
• Ensure accuracy and attention to detail in case notes, voucher coding, and reporting
• Refer participants to other internal programs and external community resources as appropriate

Position Requirements

• Belief in the mission to provide food, warmth, and shelter
• At least 1 year of experience working with vulnerable populations or evaluating eligibility and connecting individuals to resources
• Strong interpersonal, written, and verbal communication skills
• Ability to work effectively with individuals from diverse backgrounds with empathy and professionalism
• Demonstrated ability to collaborate with participants, coworkers, and community partners while maintaining confidentiality
• Proficiency in Microsoft Office and web-based applications
• Ability to manage multiple tasks and adapt to changing priorities
• Willingness to work flexible hours and in varying conditions
• Must have a work-issued cell phone (or approved alternative)
• Valid Oregon driver’s license with an acceptable driving record

Preferred Qualifications

• Experience working with HMIS or other database systems
