Referral Coordinator

Position Summary

The Referral Coordinator oversees patient referrals to ensure timely access to care. This role works closely with patients, providers, and external healthcare partners to coordinate services, manage documentation, and streamline the referral process. The ideal candidate is highly organized, detail-oriented, and committed to delivering excellent patient support.


Qualifications

  • High school diploma required; bachelor’s degree preferred
  • Minimum of 2 years of experience in referral coordination within a healthcare setting
  • Experience in primary care or medical assisting preferred

Key Responsibilities

  • Track, coordinate, and communicate all patient referrals
  • Manage care coordination activities, including referral management, care transitions, and task administration
  • Prioritize referrals based on urgency and ensure timely follow-up
  • Review referral details and expectations with both providers and patients
  • Compile relevant patient medical history and documentation to support referral needs
  • Ensure accurate and complete patient registration information
  • Verify insurance benefits and obtain prior authorizations as required
  • Communicate with insurance companies to provide necessary clinical information and establish medical necessity
  • Serve as the primary point of contact for patients, specialists, and referring providers regarding referrals
  • Respond to requests from third-party referral sources and provide appropriate information
  • Assist patients in resolving barriers to care, including financial or social challenges (e.g., transportation resources)
  • Communicate updates between referring providers and care teams
  • Maintain accurate documentation within the electronic health record (EHR) system
  • Manage referral-related faxes and distribute them to appropriate departments or patient records
  • Provide referral status updates to care teams and referring providers
  • Maintain and update internal referral tracking systems for reporting and data management

Wage: $20/hr

Location: In person. Durango, CO

Schedule: M-F. 8:00am-5pm or 8:30am-5pm
