Job Description:

• The Program Manager serves as the point of contact for the clients, family members, and caregivers.
• Must balance warmth and empathy.
• Assist PCA's and other staff with intake and admission process. 
• Manage staff scheduling with CareSmart.
• Monitor client satisfaction.
• Complete quarterly performance evaluations for PCAs.
• Monitor caregiver compliance.  
• Assist with general office organization including maintain clean and common areas, filing and supporting documents.  
• Prepare for audits.
• Maintain confidentiality.

 

 

Qualifications: 

• MUST HAVE 3+ years of home health experience or case management. 
• Familiarity of Medicaid waiver programs, EVV Systems, and home health documentation forms. 
• Associate or bachelor's degree in health care administration, social work or related field.
• Strong communication skills *Bilingual is a plus*
