Differential Response Case Manager - English/Spanish

Salary: $30-32/hr - Direct Hire Role

We are looking for a bilingual case manager to work in a community and family centered organization. We are looking for someone who has more than 2 years of experience, someone who is willing to work in community and non-profit based work. This is a full time in person position.

Minimum Qualifications & Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (preferred)
  • Bilingual Spanish proficiency (oral and written) required
  • Experience working with children and families in community-based settings
  • Knowledge of trauma-informed care and strength-based approaches
  • Valid California Driver’s License, reliable transportation, and current auto insurance
  • Ability to successfully complete background clearances, including Live Scan and Child Abuse Index

Key Responsibilities

1. Family Engagement & Case Management

  • Manage referrals from CFS for DR Path II and After Care families
  • Participate in required Transition Meetings
  • Conduct initial strengths-based family assessments
  • Develop individualized Family Service Plans (FSPs) in partnership with families

2. Direct Service & Home Visiting

  • Provide direct case management to families experiencing multiple stressors (e.g., domestic violence, substance use, mental health challenges)
  • Conduct in-home and in-person visits throughout the community
  • Work flexible hours, including evenings and weekends as needed
  • Provide hands-on support and coordinated linkage to essential community resources (e.g., housing, employment, mental health services)

3. Documentation & Compliance

  • Maintain accurate and timely case records using required DR program data-collection tools
  • Document family engagement and service contacts
  • Complete case exit plans, closure summaries, and required follow-up contacts
