Care Manager (Full-Time)

We are seeking a compassionate and organized Care Manager to support older adults and individuals with disabilities by coordinating services and developing personalized care plans. This role involves working directly with clients, families, and service providers to ensure individuals receive the support they need to live safely and independently in their communities.

Schedule: Monday – Friday 8:00 a.m. to 4:30 p.m. (No weekends, evenings, or on-call hours)

Good to Know: option of being REMOTE, but home visits with our program participants are required. And this is a Direct Hire position through Express Employment Staffing Firm. 

Key Responsibilities

• Conduct in-person assessments with clients to evaluate needs related to health, housing, support systems, and daily living

• Develop individualized care plans in collaboration with clients and their families or caregivers

• Connect clients with appropriate community resources and support services

• Monitor client progress and coordinate ongoing care with providers and support networks

• Maintain accurate documentation and reports related to client services and program requirements

• Advocate for clients and ensure services are delivered effectively

Qualifications

• Degree in social work, psychology, counseling, nursing, health services, or a related field preferred, or equivalent experience working with seniors or individuals with disabilities

• Strong communication, organizational, and problem-solving skills

• Ability to manage multiple cases and maintain accurate documentation

• Compassionate approach to working with diverse populations

• Valid driver’s license and reliable transportation required for occasional home visits
