Position Summary

 

The Referral Coordinator serves as the primary point of contact for incoming patient referrals and plays a key role in guiding patients smoothly through the admission and scheduling process. This position ensures accurate information collection, insurance verification, timely communication with referral sources and patients, and coordination with providers, intake, and billing teams to support efficient onboarding and continuity of care.

 

Roles and Responsibilities

  • Serve as the primary liaison for referral sources to coordinate all new patient referrals.
  • Communicate clearly and professionally with patients, families, referral partners, providers, and internal team members.
  • Explain available services to patients and referral sources to ensure understanding of care options.
  • Collect and document accurate patient demographic, clinical, and insurance information necessary for processing referrals.
  • Verify insurance eligibility and benefits as required.
  • Maintain an accurate, up-to-date referral log within the company software system.
  • Collaborate closely with the intake team to ensure referrals are processed promptly and efficiently.
  • Track and maintain records of current patients who are hospitalized or in rehabilitation settings.
  • Schedule telemedicine, observation, and initial appointments for new patients.
  • Manage and coordinate clinic provider schedules, including booking appointments, optimizing provider calendars, adjusting for changes, and minimizing scheduling conflicts.
  • Communicate scheduling updates or changes to patients and providers in a timely manner.
  • Work closely with the billing team to ensure complete and accurate documentation.
  • Identify and escalate barriers or delays in the referral, admission, or scheduling process.
  • Maintain confidentiality of all patient and organizational information in accordance with HIPAA guidelines.
