Position Title:    MDS Coordinator

 

Position Summary:

Assures the comprehensive assessments of residents through utilization and completion of the RAI and other assessment tools in accordance with requirements of regulatory agencies.  Interfaces with the Interdisciplinary Care Plan Team to assure completeness of findings and the development of a comprehensive care plan.

 

Essential Duties and Responsibilities:

 

• Assumes all responsibilities of the RAI/Care Plan R.N. Coordinator as outlined in the regulatory guidelines and the RAI/Care Plan Policy and Procedure.
• Follows Federal guidelines for calendar scheduling of the RAI/Care Plan process for all Medicare/non-Medicare resident assessments.
• Plans, organizes and coordinates the RAI/Care Plan schedule.
• Complies with Code of Conduct in all relationships with clients, staff members, and visitors. Adheres to the Code of Conduct in all daily activities and decisions ensuring that ethical standards and applicable laws and regulation are met. Responsible for the integrity and accuracy of organizational documents and reports. Reports violations or suspected violations of the Code of Conduct appropriately. Maintains the policy of confidentiality.
• Supports and upholds the principles of the organizational Mission, Vision and Values.

 

Education and Experience:  

• Graduate of an Accredited Professional Nursing School.
• 2-3 years clinical experience in adult health/ experience in gerontology preferred.
• Active State of Florida RN/LPN Licensure.  
• Active CPR. 
• Basic Computer and Language skills.
• Preferably 1 year experience.

• While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; and talk or hear.
• The employee frequently is required to stand, walk, climb or balance, and taste or smell.
• The employee is occasionally required to sit and stoop, kneel, crouch, or crawl. 
• The employee must regularly lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds. 
• Specific vision abilities required by this job include close vision, distance vision, color vision, and peripheral vision.
