About the Role

We are seeking a Licensed Physical Therapist (PT) to join our clinical team. In this role, you will provide high‑quality patient care focused on restoring function, improving mobility, relieving pain, and preventing physical disabilities caused by injuries, chronic conditions, or disease.

The ideal candidate is compassionate, clinically skilled, and committed to delivering evidence‑based physical therapy in a fast‑paced outpatient environment.

Key Responsibilities

Plan and administer medically prescribed physical therapy treatments to patients with musculoskeletal, neurological, joint, or bone conditions.
Review and evaluate patient conditions, physician referrals, and medical records to determine the appropriate plan of care.
Conduct comprehensive patient assessments, including strength, range of motion, balance, posture, muscle performance, respiration, and motor function.
Develop both short‑ and long‑term treatment goals and measure patient progress at regular intervals.
Prepare and modify individualized treatment programs based on patient response and ongoing evaluation.
Document all patient treatments, progress, and clinical findings in the Electronic Health Record (EHR).
Write and update monthly progress notes while maintaining accurate chart documentation.
Educate patients on home exercise programs and the proper use of assistive devices.
Provide discharge planning and communicate effectively with referring physicians and healthcare team members.
Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides involved in patient care.
Collaborate with the interdisciplinary team to ensure coordinated, high‑quality rehabilitation care.
Complete all required company orientation, meetings, and continuing education as needed.
Follow HIPAA, OSHA, and all federal, state, and local regulations.
Adhere to company policies and perform other related duties as assigned.

Qualifications

Valid Physical Therapist License in the state of practice.
Doctorate (DPT) or Master's in Physical Therapy required.
Strong interpersonal and communication skills, with the ability to build positive relationships across all levels.
Ability to thrive in a fast‑paced, dynamic clinical environment.
Dependable, punctual, and committed to high‑quality patient care.

Working Conditions

Ability to work evenings and weekends as needed.
Frequent standing and walking required for long periods of time.
Must be able to move heavy equipment and assist with patient transfers; ability to lift up to 100 lbs required.
