Position Overview

The Care Coordinator will serve as the primary point of contact for clients, families, and service providers. This role ensures that care plans are followed, services are scheduled, and communication flows seamlessly across all parties. The ideal candidate is a strong communicator with a passion for helping others and excellent organizational skills.


Key Responsibilities

  • Conduct intake assessments and gather relevant client information.

  • Develop, update, and monitor individualized care plans.

  • Coordinate appointments, services, and referrals with providers.

  • Maintain frequent communication with clients and families to ensure needs are met.

  • Track progress, document updates, and manage case records accurately.

  • Work collaboratively with multidisciplinary teams to support client outcomes.

  • Assist with crisis intervention and problem resolution when needed.

  • Ensure compliance with company policies, confidentiality standards, and regulatory requirements.


Qualifications

  • Bachelor’s degree in Social Work, Nursing, Psychology, Human Services, or related field (or equivalent experience).

  • Previous experience in care coordination, case management, or client support preferred.

  • Strong communication, interpersonal, and advocacy skills.

  • Excellent organizational and time-management abilities.

  • Ability to work with diverse populations with empathy and respect.

  • Proficiency in electronic health records (EHR) or case management systems (preferred).

  • Valid driver’s license and ability to travel locally if required.
