Job Title: Case Manager (Evaluation-to-Hire)

Pay: $19.53/hour
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Work Environment: Primarily Remote
Classification: Evaluation-to-Hire

We are seeking a compassionate and detail-oriented Case Manager to provide support to clients seeking assistance through CERA and related programs. This role offers meaningful work helping individuals access critical resources while building a career in social services.

Key Responsibilities:

  • Provide one-on-one case management and client support services
  • Conduct assessments to determine client eligibility for programs and services
  • Coordinate internal and external referrals and resources
  • Assist clients with documentation, application completion, and follow-up
  • Create award vouchers and process payments to landlords and utility providers
  • Accurately enter and maintain client data in the agency database
  • Participate in outreach efforts to promote available programs and services

Qualifications:

  • Strong organizational and communication skills
  • Ability to manage multiple client cases and meet deadlines
  • Detail-oriented with experience in data entry and documentation
  • Professional, empathetic, and client-focused approach

Equipment Provided:

  • Company-issued laptop and phone

Additional Requirements:

  • Valid driver’s license, current auto insurance, and reliable vehicle required for occasional travel
