Member Wellness Coordinator

Location: Grants Pass, OR

Express Employment Professionals is recruiting for a company in Grants Pass seeking a Member Wellness Coordinator. This position supports health education, wellness programs, and community outreach to help health plan members make positive lifestyle changes.

What You’ll Do

• Coordinate and facilitate wellness programs focused on tobacco cessation, weight management, oral health, and maternal, child, and youth health.

• Engage members through phone calls, individual coaching, group meetings, classes, and community events.

• Help members with chronic conditions and special healthcare needs work toward their health goals.

• Collaborate with care coordination teams and community partners to connect members with resources.

• Plan community wellness events, including logistics, staffing schedules, materials, and follow-up.

• Maintain accurate electronic health records, spreadsheets, inventory logs, and program reports.

• Track member participation and help evaluate program effectiveness.

• Protect confidential information and follow HIPAA requirements.

What We’re Looking For

• High school diploma or GED.

• Two to four years of related experience and/or training in group facilitation and electronic health records.

• Familiarity with the healthcare industry.

• Strong communication, organization, customer service, and computer skills.

• Confidence speaking to groups and working with people from diverse backgrounds.

• Ability to manage competing priorities, work independently, and collaborate with a team.

• Valid Oregon driver’s license and vehicle insurance.

• Current Basic Life Support (BLS) certification or willingness to obtain it within 90 days of hire.

• Current certification as a tobacco cessation program facilitator, including Dimensions Tobacco Free, or willingness to obtain certification within 90 days of hire.

• Tobacco Treatment Specialist certification preferred.

Work Environment & Requirements

• Hybrid office and work-from-home responsibilities; must be available to work onsite as needed.

• Local travel and occasional evening or weekend work for community events.

• Personal vehicle use may be required, with mileage reimbursement.

• Personal cell phone use may be required, with an applicable stipend.

• Occasional lifting or moving of up to 25 pounds, with reasonable accommodations available.
