Requirements

• Licensed Physical Therapist
• One year of recent Physical Therapy experience, preferably in home health or another relevant setting (rehab, acute care, outpatient)
• Current BLS/CPR certification
• TB test 

Responsibilities

• Evaluate patients’ physical conditions, mobility, strength, balance, and pain levels to develop individualized care plans that promote functional independence at home.

• Create and implement personalized therapy programs focused on restoring movement, relieving pain, and preventing disability or further injury.

• Provide hands-on care, therapeutic exercises, gait training, balance activities, and use of assistive devices to improve mobility and overall quality of life.

• Instruct patients and family members on safe exercise techniques, body mechanics, and use of adaptive equipment to support ongoing recovery and independence.

• Maintain accurate, timely, and compliant clinical records. Communicate regularly with physicians, nurses, occupational therapists, and other healthcare team members to ensure coordinated care.

• Continuously assess patient progress and modify treatment plans as needed to achieve optimal outcomes.

• Adhere to all regulatory and professional standards for home health care, ensuring patient safety and confidentiality at all times.
