Position Summary

The Transitional Care Management (TCM) Outreach Coordinator is responsible for contacting eligible patients following hospital or facility discharge to facilitate enrollment in the clinic’s Transitional Care Management program. This role supports continuity of care by conducting patient outreach, providing education about services, coordinating follow-up appointments, and accurately documenting all interactions.

The ideal candidate is detail-oriented, organized, and comfortable communicating with patients by phone while performing accurate data entry and tracking.

 

Key Responsibilities

  • Conduct outbound phone calls to eligible patients to explain and enroll them in the Transitional Care Management program
  • Educate patients on program benefits, services, and follow-up care expectations
  • Verify patient information and update demographic and insurance details as needed
  • Schedule follow-up appointments and coordinate care with providers
  • Document all patient communications and enrollment activities accurately in the EMR
  • Track patient status, outreach attempts, and program participation
  • Maintain accurate census lists and reporting logs
  • Collaborate with providers, intake, and care teams to ensure seamless transitions of care
  • Follow HIPAA guidelines and protect patient confidentiality
  • Assist with additional administrative or data-entry tasks as assigned

Qualifications

  • High school diploma or equivalent required; healthcare or medical office experience preferred
  • Experience in patient outreach, customer service, or call-based roles preferred
  • Strong communication and phone skills
  • Detail-oriented with excellent organizational skills
  • Comfortable with data entry and electronic medical records (EMR systems)
  • Ability to multitask and manage time effectively
  • Compassionate and professional demeanor when working with patients
