Job Summary
RN or LPN is responsible to provide professional nursing care to our residents oversee CNAs in their day-to day assignments. Carries out the nursing process through assessment, planning, implementing, documenting, and evaluating plans of care for assigned residents. Collaborates, as appropriate, with physicians and other health care teams regarding resident needs.
Essential Job Functions
- Provides nursing care in accordance with the State of Hawaii Nurse Practice Act and State Department of Health Regulations.
- Maintains compliance with all personnel policies, established facility policies and procedures.
- Understands and implements the fire, safety, and disaster preparedness plan and evacuation procedures of the facility during drills and emergencies.
- Understands the resident’s Advance Directives and follows his/her wishes.
- Assists residents in scheduling medical, dental, and laboratory appointments.
- Reviews and transcribes physician’s orders.
- Monitors resident’s restorative nursing programs.
- Supervises and monitors medications for residents who self-administer.
- Assist with assignment making for team members and follow-up to ensure high standards of care and documentation of such care.
- Performs admission and periodic assessments on residents for functional and mental abilities.
- Reviews and revises assessments and care plans at admission and at change of condition.
- Assists physician with resident examination as needed.
- Administers and documents medications in accordance to the six rights; right resident, right medication, right dose, right time, right route, and right documentation/record.
- Provides medical treatments as ordered by physician.
- Assists resident with meeting their nutritional/hydration needs and documents on the health record appropriately.
- Interpret and evaluate diagnostic tests to identify and assess resident condition.
- Consult and coordinate with healthcare team members to assess plan, implement or evaluate resident care plans.
- Observes residents and reports condition changes to physicians and at change of shift reporting.
- Coordinates resident care between other disciplines, co-workers on different shifts, physicians and ancillary services providers.
- Demonstrates accurate head to toe physical assessment skills for initial and follow-up appraisal of resident status.
- Possesses knowledge of clinical disease entities.
- Demonstrates the ability to administer medications and to perform treatments and procedures as prescribed and document in an appropriate manner.
- Demonstrates knowledge of specialized diets and lab work.
- Demonstrates knowledge and awareness of rehabilitative and preventive aspects of care that ensures resident safety, comfort and protection, with the ability to initiate corrective action immediately.
- Assists physicians in making rounds and communicates changes of condition of residents based on ongoing assessments.

- Reports diagnostic data and incorporates it into the plan of care.
- Assists in providing resident and family teaching and training.
- Identifies resident problems and emergency situations and initiates “life-saving” measures.
- Assures that medications are securely stored.
- Assures that medication cart is locked at all times except when in active use.
- Maintains current knowledge of facility policies and procedures and their implementation.
- Maintains infection control practices as outlined in the facility policies and procedures.
Work Experience Requirements/Skills
- One year experience or equivalent as a LPN, or RN in a Nursing Home, Care Home, or Hospital. ▪ Ability to effectively communicate with co-workers, residents, Physicians and Management ▪ Project Management Skills
Educational and Professional Requirements
▪ Graduation from an accredited School of Nursing ▪ Current State of Hawaii License as a Licensed Practical Nurse or Registered Nurse ▪ Current CPR card
