Position Summary:

The Healthcare Coordinator for the Developmental Disability Waiver at Express Employment (Express Healthcare) plays a crucial role in supporting Home and Community Based Services (HCBS). These services are designed to assist individuals with diverse needs, enabling them to remain independent and improve their quality of life within their homes and communities. This position ensures that eligible individuals receive essential services to help them live outside institutional settings.

After 3 months, you able to opt into the commission which adds approximately $1.68/hr to your base pay!

Key Responsibilities:

Processing and Administration:

  • Process all Prior Authorizations (PAs), including annual reviews and amendments.
  • Handle the setup of new clients and discharge of clients, maintaining accurate records in database.

Meetings and Training:

  • Attend all in-person and virtual provider meetings.
  • Conduct timesheet training with caregivers.
  • Create and update internal training materials and provide internal training sessions.
  • Develop sample timesheets for caregivers
  • Assist with onboarding new caregivers for Developmental Disabilities.

Testing:

  • Ensure all caregivers are up to date on CPR, First Aid, and additional state testing.

Communication and Coordination:

  • Maintain regular communication with case managers via phone calls.
  • Help create and update educational materials for caregivers.
  • Maintain and update TextUs daily.
  • Assist with DD projects throughout the year.

Database and Documentation:

  • Assist in the creation and updating of spreadsheets for the upcoming year.
  • Support DD audits as needed.

Miscellaneous:

  • Answer phones and forward calls to the appropriate personnel.
  • Provide additional support as needed for end-of-year changes and new initiatives.

About Develop Mental Disability Waiver:

Developmental services under the 0208 waiver (often referred to as a Medicaid Home and Community-Based Services (HCBS) waiver) provide support for individuals with developmental disabilities to live more independently in their communities. These services may include case management, personal care assistance, respite care, supported employment, day programs, and other specialized therapies. The goal is to help participants maintain and enhance their quality of life while reducing the need for institutional care, promoting inclusion, and ensuring access to necessary support.
