Care Manager: 

 

We''re seeking a dedicated Care Manager to join our team. As a Care Manager, you will play a crucial role in providing ongoing support and casework for Aging programs and services. Your responsibilities will include conducting comprehensive client assessments, developing personalized care plans, coordinating support services, and advocating for clients'' needs.

 

Key Responsibilities:

• Conduct comprehensive client assessments in various community settings.
• Develop personalized care plans in collaboration with clients and caregivers.
• Coordinate and link clients to suitable support services.
• Monitor and review client progress, providing ongoing care coordination and advocacy.
• Ensure compliance with program regulations and documentation requirements.
• Collaborate with agency staff members and attend relevant meetings.
• Maintain certification as an Indiana Case Manager and participate in continuing education activities.

Qualifications:

• Registered nurse, licensed practical nurse, or bachelor''s/associate''s degree with relevant experience. Master''s Degree will be considered in place of relevant experience).
• Strong organizational and multitasking abilities.
• Proficiency in computer skills and willingness to learn new systems.
• Excellent communication skills for effective interaction with clients and agencies.
• Ability to work with clients in a supportive and non-judgmental manner.
• Capacity to adapt to change and maintain confidentiality.
• Reliable transportation and willingness to travel to client locations.

Physical Requirements:

• Ability to perform essential job functions, including sitting, standing, bending, and manual dexterity.
• Capability to visit various home environments, potentially involving walking on rough terrain and climbing stairs.
• Normal range of hearing and vision.
• Ability to lift up to 15 pounds.

Direct Hire

Monday-Friday, 8:00AM-4:30PM (37.5 hours/week).
