Case Manager

Located in PHILADELPHIA, PA

Salary: $41,209

Express Employment Professionals has been retained by a non-profit client to hire a full time case manager. Our client provides services to persons experiencing partner violence and works to end domestic violence through advocacy and community education. This position is a Case Manager for Long Term Housing and provides continued supportive housing counseling, aftercare housing counseling and advocacy to survivors of domestic violence. 

Duties -

1. Provides trauma informed supportive housing counseling and advocacy 
2. Provides individual case management, advocacy, needs assessment, action planning and evaluation for assigned caseload.
3. Accompanies residents to appointments as necessary for supports, advocacy and obtaining and maintaining safe independent housing.
4. Develops, implements, coordinates and facilitates group meetings which address personal and emotional needs of the family.
5. Provides in home and telephone, ongoing individual support and informal counseling and makes outside referrals when appropriate.
6. Provides training and education

Qualifications -

1. Bachelor’s Degree in social services or related field,
2. Intermediate computer skills, including MicroSoft Office Suite (Word, Outlook, Excel) 
3. Demonstrated commitment to service of others is required.
4. Demonstrated proficiency in communication, both verbal and in writing, is required. 5. Demonstrated ability to work effectively with a diverse population is required, experience working in with diverse populations and in an urban setting strongly preferred.
6. Demonstrated ability to work effectively as a team member is required.
7. A valid driver’s license required and own vehicle is preferred.

MUST submit and retain satisfactory clearances (child abuse, criminal and FBI clearances) pre-employment and ongoing employment required
