Express Healthcare Professionals is currently seeking a dedicated RN Case Manager for a local Hospice Care company in Honolulu. This role requires a compassionate and skilled nurse to provide exceptional care to patients and their families, both in-home and via telehealth. For more information please call, 808-792-7000. 

 

• In-Home and Telehealth Support: Directly provide in-home support to patients, families, and the palliative care team of community-based physicians and nurse practitioners. Offer telehealth (video and audio) support when appropriate.
• Patient Assessment: Conduct telehealth patient assessments prior to initial visits and between subsequent visits. Assess patients' physical and mental health, including reactions to illnesses and treatment regimens.
• Home Visits & Monitoring: Provide home visits and telehealth monitoring as needed to support the Integrated Care team. Monitor patient acuity to ensure they receive the right care at the right time and place.
• Chronic Care Coordination: Identify and coordinate the needs of chronically ill patients and families, supporting clinicians to operate as efficiently as possible.
• Electronic Records: Record and report the results of nursing assessments in the electronic medical record system.
• Collaborative Care Planning: Collaborate with other healthcare providers to develop patient and family-centered nursing care plans. Ensure the implementation of these plans is well-coordinated.
• Treatment Implementation: Implement prescribed treatment and pharmaceutical regimens as authorized by State law.
• Patient Education & Counseling: Provide teaching and counseling on health, disease progression, and symptom management.
• Care Documentation: Report and document the care plan, nursing care provided, and patients' responses to care.
• Safety & Effectiveness: Ensure the maintenance of safe and effective nursing care, whether delivered directly or indirectly (by unlicensed staff, patients, and families).
• Advance Care Planning: Facilitate discussions on advance care planning.

Requirements:

• Bachelor of Science in Nursing (BSN) from an accredited school (preferred).
• Valid Hawaii State RN license.
• Minimum of 2 years of clinical RN experience.
• Current CPR/BLS and First Aid Certification.
