Express Employment Professionals is looking to add a Shelter Case Manager to our Stuart, FL team! 

 

Primary Objective: 

Responsible for providing Trauma Informed Care, to help families experiencing homelessness, often living in hotels or with friends or family, remain in a safe, secure environment while assisting them to secure their own housing.  This organized, compassionate, and motivated person will quickly transition families from homelessness to permanent housing, following a rapid-rehousing, housing-first approach.  The CM will also assist families in obtaining outcomes such as securing employment, increasing earning potential, securing/maintaining housing, and securing a trade/certificate. 

 

Responsibilities:

  1. Exhibits a professional attitude and models’ appropriate attire, communication and personal interaction towards guest and host sites.
  2. Conducts phone intakes and reports to Case Management Director (CMD).
  3. Lead the screening and intake process for all potential families: conduct intake interviews including talking with referral source(s)/partners; conduct orientation for new families moving into the shelter and transitional housing program.
  4. Communicate with the Host & Support Coordinators to ensure hosting congregations have necessary family information (work schedule, food allergies, vehicle, etc.).
  5. Find housing for families by liaising with landlords and getting families ready as needed: debt repair, ensuring families can get utilities turned on in their name, will have furniture, etc.
  6. Work with Case Management Director and Case Manager program assistant to deliver resources, referrals and workshops as needed related to employment, financial literacy and budgeting, housing plans, and educational planning.
  7. Develop relationships with partner organizations and maintain database of resource for families.
  8. Serve as an advocate for families with community service agencies.
  9. Provide crisis support to families and volunteers as needed.
  10. Caseload includes 10 – 20 families in network/in-house programs and est. 25-35 graduate families at any given time.
  11. Additional responsibilities include: Weekends, evenings, assisting in Prevention, Diversion, Almost Home, Open Table or Program Nights.

 

Qualifications:

  1. Shelter Experience and/or experience with working with the Homeless is a Strong PLUS.
  2. At least two years of experience in the not-for-profit field.
  3. Proficient in basic computer applications such as Microsoft Word, Excel, Outlook, e-mail and internet usage
  4. Record keeping, organization and planning skills
  5. Demonstrated capability to conduct self in a calm and professional demeanor when dealing with guests and difficult situations
  6. Demonstrated capability to effectively communicate orally and in writing
  7. Ability to work well with a diverse group of guest and volunteers
  8. Ability to effectively manage a wide array of tasks, projects and responsibilities
  9. Ability to work productively in an unstructured environment with frequent interruptions
