One of Oregon’s rapidly growing in-home care organizations is seeking an experienced Care Manager to lead their Central Oregon team!

 What You Get to Do!

The Care Manager (CM) is responsible for all aspects of Quality Care for Clients and Caregivers, including training, management, record-keeping, mentoring, retention and discipline of Caregivers, and quality care assurance for Clients. The CM will be evaluated on the quality of the Caregiving workforce, as well as on their dedication to onsite Client quality of care evaluations and quality case management. The CM will report to the Executive Director. The performance pillars for the Care Manager are a focus on Caregiver Orientation and Training (25%), Caregiver Retention and Management (25%), Client Quality Care Evaluations (25%), and Industry Compliance (25%).

          Preparing for and hosting Caregiver Orientation for new hires

          Ensuring complete and accurate files for all Caregiving staff

          Ensuring identification documents are physically verified for I-9 completion

          Ensuring that only fully background checked and trained Caregivers are on active shifts

          Providing regular in person touch points for Caregivers at 90 days post-hire and throughout their employment

          Encouraging strong retention rates through ongoing mentorship and positive, intentional leadership

          Surveying new Caregivers post-orientation

          Conducting Caregiver Annual Performance Reviews and making recommendations for compensation adjustments

          Reconciling and tracking training requirements for initial training requirements and for annual CEU’s

          Developing and implementing Caregiver Improvement Plans

          Assisting in Work Comp and Unemployment claims processing as needed

          Assisting in Verifications of Employment

          Offboarding Caregivers for resignations and terminations

          Ongoing coordination with Community Referral Partners – including providing EVV documentation to DHS

          Ongoing coordination with Medicaid Case Managers

          Field Work – Assisting in intaking new Clients at initial in home consultation

          Field Work – Assisting in development of robust and comprehensive Care / Service Plans to provide customized care for each Client, according to their individual needs, preferences, and Payee scope of authorized/allowable services

          Field Work – Performing 7-30-day (initial) and 90-day (quarterly) on site evaluations of existing Clients

          Current Client and Family Member Relations

          Casework management of Current Clients
